NTS identified all ACS cases, while MHS and INTERCHEST each missed 2 of 13 ACS cases. Other strategies using point-of-care troponin testing or ECG may improve accuracy but are unavailable during initial telephone triage. All three evaluated tools failed to show reliable discrimination for ACS in this sample. Acute coronary syndrome encompasses unstable angina, NSTEMI, and STEMI, all caused by acute myocardial ischaemia from coronary plaque rupture or erosion. ACS occurred in 13 of the 280 analysed patients. ACS was one of the primary reference outcomes assessed within 6 weeks.
