Caesarean sections were often performed by non-physician clinicians in Malawi and Tanzania but only by doctors in Uganda and Benin. The participating countries shared some system characteristics but differed in maternity care organization and workforce arrangements. ALERT was implemented in 16 district-level hospital maternity units across Benin, Malawi, Tanzania, and Uganda. Each participating hospital had at least 2,500 births per year when the project began. ALERT was a quality improvement project launched in 2020 to strengthen intrapartum care through participation, training, mentoring, and quality improvement activities.
