Inpatient APEX increased slightly from 2001 to 2023 despite work-hour restrictions. Emergency department APEX decreased between 2007 and 2023 even though total ED volume increased. Annual Patient Exposure was defined as annual patient volume divided by resident number and used as a proxy for resident clinical exposure. APEX is presented as useful for planning workload and medical education but not as a substitute for direct training-quality measures. APEX was calculated separately for inpatient wards and the emergency department.
