First-trimester folate supplementation was low among mothers of affected births. First-trimester antenatal care attendance was low among mothers of affected births. The article identifies early antenatal care and periconceptional folate or multivitamin supplementation as major prevention targets. The public health response should improve access to early antenatal care and periconceptional supplementation.
