Anterior ankle pinching is commonly noticed after inversion ankle sprain or during dorsiflexion-heavy movements such as squatting. Inversion sprain mechanics may move the talus anteriorly and sensitize tissues at the front of the ankle. A forward center of mass and plantar flexor tone can maintain anterior talar glide during dorsiflexion. Trunk and pelvic organization are presented as the first intervention priority before local ankle techniques. Local ankle treatment is considered after trunk, pelvis, and hip factors have been addressed. Heel elevation can be a pragmatic option when structural limitations prevent comfortable dorsiflexion.
