Anti-gliadin IgA is not a diagnostic marker for celiac disease. Anti-gliadin IgA on the GI Map represents mucosal immune response to gluten, specifically gliadin. Anti-gliadin IgA measures activity of secretory IgA binding to gliadin rather than total secretory IgA count. Anti-gliadin IgA may also be elevated in undiagnosed celiac disease or in diagnosed celiac patients exposed to gluten. A very high anti-gliadin IgA result with low secretory IgA makes it prudent to rule out celiac disease urgently. Anti-gliadin IgA is primarily used to indicate non-celiac gluten sensitivity.
