The intervention produced modest reductions in blood pressure and improved physical activity and salt intake, but did not improve medication adherence or global dietary scores. Home visits promoted blood pressure self-monitoring, but many participants needed additional support. Alcohol reduction improved in the intervention arm, while smoking did not change overall. Stopping bad behaviors can naturally produce significant progress toward health and wellness. Participants experienced five programme elements as supporting behaviour change.
