The home visit manual was adapted for Nepali-language use and supplemented with family engagement. The CAHM intervention was evaluated as a community health worker-facilitated home visit intervention within a cluster randomised trial in urban Nepal. The intervention targeted medication adherence, follow-up care, blood pressure self-monitoring, lifestyle behaviours, and reduced alcohol and tobacco use.
