Care Coordination

A central coordinator could reduce duplication and prevent inconsistent messages to patients. Participants proposed centralised care coordination by a professional with broad oversight of the patient trajectory. Task delegation was proposed as a response to workforce shortages. The coordinator role may face barriers such as lack of time, authority, compensation, and current care agreements. General practitioners and occupational physicians were most often identified as possible care coordinators.
