CHARMER

The intervention targets clinician capability, opportunity, and motivation to identify deprescribing opportunities and discuss them with older adults in hospital. CHARMER was developed using deprescribing evidence, behaviour change theory, and co-design with geriatricians and pharmacists. CHARMER is a hospital deprescribing programme for older adults admitted to older people’s medicine wards in England. CHARMER was designed to test whether the intervention is effective, cost-effective, and safe. CHARMER is delivered to healthcare professionals rather than directly to patients.
