Standard right hemicolectomy does not expose the SMA and SMV or ligate the relevant vessels at their origins. Complete mesocolic excision with central vascular ligation was described as a plane-based oncological resection technique analogous to total mesorectal excision. CME requires sharp embryological-plane dissection, preservation of the mesocolic fascial envelope, en bloc right mesocolon removal to the SMV, and central vessel ligation. CME is technically more demanding than standard right hemicolectomy because central dissection near superior mesenteric vessels creates vascular injury risk.
