Clinicians debated whether COVID-19 ARDS should be ventilated differently from other ARDS because physiology varied widely and recommendations were uncertain. CAVIARDS was initially designed for COVID-19 ARDS and later changed to a basket design after COVID-19 incidence fell. Early observational studies from China and Germany estimated COVID-19 ARDS mortality at approximately 60%. In the CAVIARDS basket design, CAVIARDS-19 enrols suspected COVID-19 ARDS and CAVIARDS-all enrols non-COVID ARDS, with the primary analysis performed separately for each subpopulation.
