Under the unconditioned prompt condition, the strongest population-level DR counterfactual effect came from tortuosity_4x. Benfotiamine has shown benefit for both peripheral neuropathy and diabetic retinopathy. A ketogenic diet combined with intermittent fasting can potentially reverse some retinopathy damage if the condition has not progressed too far. Under the DR-prompt condition, the tortuosity_4x effect was smaller than under the unconditioned condition. The eyes are the most susceptible tissue to glycation damage, ranking above peripheral nerves and arteries. The diabetic retinopathy analysis found that top SHAP-ranked Bézier features aligned with known vascular directions in DR. The tortuosity_4x effect reproduced across EfficientNet-B2 and ResNet-50 backbones and across prompt conditions. Whether advanced diabetic blindness can be reversed once it has occurred is uncertain. Diabetic retinopathy can progress without symptoms in its early stages. The absence of symptoms does not mean diabetic retinopathy is absent.
