The application of DRGs to emergency department settings remains largely unexplored. The concentration of ED presentations in a small number of ICD-10 codes supports both the feasibility and necessity of implementing ED-specific DRGs in Malaysia. DRGs originated in the United States in the early 1980s and have since been adopted globally for inpatient care. DRGs classify hospital cases into groups expected to consume similar resources, standardising reimbursement, supporting financial planning, and improving cost predictability. Emergency departments present distinct challenges for DRG implementation due to diverse acute presentations, limited initial clinical information, and variable resource use. Standardising ICD-10 documentation across Malaysian EDs is a prerequisite for any DRG framework, not merely a parallel effort.
