Co-production and power-sharing, where people with disabilities have genuine influence over research decisions rather than merely consultative roles, are critical structural components of disability-inclusive trials. People with disabilities were included as co-authors and participated in the design, implementation, dissemination, and reporting of this scoping review. People with disabilities must be integrated into clinical trials not only as participants but as investigators, advisors, and collaborators across all phases of the trial process. Research teams should create training and mentorship pathways for people with disabilities, explicitly valuing lived expertise.
