Sham stimulation uses the same placement and device but ramps current up briefly before decreasing it to off. Active tDCS is delivered for 20 minutes at 2 mA using two 5 x 5 cm electrodes. Prior meta-analytic evidence did not show significant benefits of anodal tDCS over sham for pain or disability in chronic low back pain. The DLPFC is considered a promising stimulation target because it may enhance endogenous pain modulation. Active stimulation places the anode over the left prefrontal cortex and the cathode over the right supraorbital area.
