Eisenmenger syndrome is described as the advanced end of pulmonary arterial hypertension associated with unrepaired congenital heart disease. Evidence for existing pharmacotherapy in Eisenmenger syndrome is limited and mixed. Progressive pulmonary vascular remodelling can reverse shunt flow and cause cyanosis and multiorgan complications. Many adults with Eisenmenger syndrome or vasodilator-resistant unrepaired CHD-PAH are not candidates for defect closure.
