Prospective data confirming the safety and fertility effectiveness of sclerotherapy are absent. Retrospective observational studies of sclerotherapy in large endometriomas reported a moderate AMH decline and a 57% pregnancy rate at 24 months. The trial uses a standardized 10-minute, 10-step ethanol sclerotherapy protocol. The ATOPE observational arm treats endometriomas larger than 6 cm with laparoscopic sclerotherapy. Ethanol sclerotherapy has emerged as a promising minimally invasive alternative for endometriomas larger than 6 cm.
