The external obliques run diagonally from the lateral iliac crest to the lower ribs, and gain mechanical advantage when the diaphragm flattens. In narrow IFA presentations, the external obliques pull the ribcage anteriorly and externally rotate the ribs, decreasing lateral thorax dimensions. Forceful exhalation worsens narrow IFA by recruiting the external obliques and increasing compressive force on the ribcage. Activating the internal obliques and transversus abdominus counteracts the external oblique-driven external rotation by internally rotating and depressing the ribs.
