HLA-DRB1*0301 is associated with a benign, self-limited non-progressive sarcoidosis course in Löfgren syndrome. These HLA genotype associations do not generalise well to diverse admixed US populations. In Swedish patients with Löfgren syndrome, HLA genotype predicts clinical outcome. DRB1*0101, *0401, *1301, and *1501 carriers more often develop progressive sarcoidosis requiring therapy.
