AYA cancer survivors were significantly more likely than cancer-free peers to meet the composite gynaecological follow-up standard, with a compliance rate of 23.2% versus 16.5%. Despite higher relative compliance, absolute gynaecological follow-up rates remained low in both groups, with the composite rate reaching only 23.2% even among cancer survivors. Cancer survivors were considerably more likely to attend at least one annual gynaecological consultation (39.1% vs 28.3%), partly because treatment-related concerns generate appointment activity that may coincide with preventive consultations. The study recommends formal integration of gynaecological management into long-term follow-up programmes for AYA cancer survivors as a structural solution to low compliance. Proposed strategies to improve gynaecological follow-up compliance include written information resources, digital monitoring tools, and professional referral networks.
