Two mechanisms may explain the free-care satisfaction paradox: perceived lower quality due to zero cost, and structural association with longer waits and less physician time. Patients receiving free-of-charge care were 77% less likely to report satisfaction than those paying out of pocket, contradicting findings from several other Ethiopian studies. Payment modes among study participants included out-of-pocket (53.3%) and health insurance (42.2%).
