Hypoglycemia

A prior pilot study showed that fully closed-loop therapy improved HbA1c without increasing hypoglycaemia risk compared with standard therapy. Hypoglycemia means low blood sugar. People with type 1 diabetes spent an average of 1.5 hours per day at or below 70 mg/dL in a sensor-based study. The low point after insulin overshoot is hypoglycemia. Recurring hypoglycemia can be driven by repeated blood sugar spikes followed by crashes. Hypoglycemia is presented as a way someone can be short on available energy without outright starvation. An insulin response to sugar and carbohydrates can overcompensate and rapidly lower blood sugar into hypoglycemia. Hypoglycemia is associated with cravings for carbohydrates, sugar, bread, snacks, and junk food. The increased mortality observed in the ACCORD trial's intensive arm was attributed to excess hypoglycaemic events. The mechanism is insulin overshoot: a large sugar intake triggers a large insulin release that drives blood glucose below hypoglycemic levels.
