Paired blood samples will assess innate and adaptive immune responses through high-dimensional single-cell profiling. The study will examine circulating immune cell populations to define lymphocyte and myeloid subset composition and activation state. Peripheral blood Tregs are profiled with single-cell captures and paired TCR VDJ sequencing to map clonotypes. The study will characterize antigen-specific T cells to evaluate cellular immune responses to RSV. Spatial biopsy features will be prospectively linked to IVUS-measured CAV progression between months 3 and 12. The study will monitor vaccine-enhanced disease by comparing hospitalised breakthrough cases with unimmunised infants with and without RSV. The immune profiling substudy will include 75 infants from the effectiveness study and 50 healthy community controls.
