Evidence for elevated intraluminal pressure as a causative factor in diverticular disease has been inconsistent. The broader literature on intraluminal pressure is described as sparse and methodologically heterogeneous. A contemporary high-resolution manometry study by Jaung et al. did not find a statistically significant pressure difference. Studies from the 1960s identified exaggerated pressure responses in diverticulosis patients compared with controls under gastrocolic reflex or pharmacological stimulation conditions. The DIPHYSIO investigators extend the intraluminal pressure hypothesis by focusing on defecatory technique.
