Dexmedetomidine-induced sedation preserves arousability and resembles natural sleep more than other hypnotics. Intravenous dexmedetomidine may improve postoperative sleep but the evidence is mixed. Dexmedetomidine has sedative, anxiolytic, sympatholytic, and analgesic-sparing effects. Dexmedetomidine promotes sleep through α2 receptor activation in the locus coeruleus. Hypotension and bradycardia commonly limit perioperative IV dexmedetomidine, especially in elderly patients.
