Complete clinical cure after surgery is uncertain because only a minority of patients stop all antihypertensive medication. Surgery may cure or substantially improve primary aldosteronism and may reduce cardiovascular and stroke risk more effectively than medical treatment. Laparoscopic adrenalectomy removes the whole adrenal gland for a disease often caused by a small benign lesion. Laparoscopic adrenalectomy is the conventional treatment for unilateral primary aldosteronism. Participants who fail ablation biochemically at 6 months may be offered unilateral adrenalectomy as standard treatment after multidisciplinary review.
