Adding linezolid to any target drug consistently increased adverse event risk signals, especially hepatotoxicity, hematologic toxicity, and peripheral neuropathy. Pretomanid-associated pancreatitis and acute kidney injury emerged as high positive signals only when pretomanid was combined with linezolid. Peripheral neuropathy in BPaL regimen studies was mainly attributed to linezolid rather than to the novel MDR-TB agents alone. For BPaL or similar regimens, the article recommends monitoring liver function, complete blood counts, and nerve conduction velocity.
