A single postoperative dose of liposomal bupivacaine can provide analgesia for up to 72 hours. Traditional local anesthetics such as ropivacaine often have insufficient duration for prolonged preoperative waiting periods in hip-fracture patients. Liposomal bupivacaine can extend analgesia through femoral or fascia iliaca blocks, but those approaches may cause prolonged lower-limb numbness and motor impairment. The trial administers liposomal bupivacaine as a 20 mL, 266 mg single injection for the PENG block. Evidence specific to open elbow arthrolysis remains scarce. The safety of liposomal bupivacaine in frailer elderly populations remains unclear. Prior studies in distal radius fixation, shoulder surgery, and shoulder arthroplasty support the rationale for testing liposomal bupivacaine in this setting. Liposomal bupivacaine is an ultra-long-acting local anesthetic based on DepoFoam technology. Liposomal bupivacaine forms a local depot that slows bupivacaine release. Liposomal bupivacaine is white or off-white and has greater injection resistance than saline, creating blinding challenges that the protocol addresses with opaque syringes and extension tubing.
