Some long-term care hospital patients remain hospitalized despite discharge eligibility because community care resources are lacking. More than half of patients discharged from long-term care hospitals are readmitted, mainly because community home care services are insufficient. Nearly all LTCI beneficiaries have chronic disease, yet chronic disease management gaps remain.
