Longitudinal SMR patterns suggested persistent relative differences even without annual outlier flags. ICUs can repeatedly sit near the high or low side of annual funnels while staying within control limits. The displayed high-end and low-end ICU examples all remained within the widest 99.8% limits. Persistent positioning near the high or low side of the funnel can indicate systematic mortality differences without an annual outlier flag.
