Medication discrepancies were detected by comparing pharmacist-collected medication histories with physician medication lists and emergency department orders. Medication reconciliation compares a patient's actual medication regimen with medication orders written during a care transition. The best possible medication history in this study included prescribed and non-prescribed medicines with medication names, doses, frequencies, routes, and durations. At least two medication information sources were used for every participant.
