Before this study, no long COVID care model had been developed specifically for older adults. The study co-designed a tailored care model for older people with long COVID. The proposed model includes assessment, coordinated multidisciplinary care, self-management education, mental health support, validation, discharge planning and research participation. Comprehensive assessment should identify symptoms, comorbidities, functional limitations, cognitive issues, mental health needs and social vulnerabilities. The care model should be adaptable because older people with long COVID are heterogeneous.
