Multimorbidity Care

Staff wanted multimorbidity care to be more personalised, holistic, and efficient. A multimorbidity template may support more efficient and personalised care when training, workload, administrative systems, incentives, and skill mix are addressed. Healthcare for long-term conditions is usually organised around single diseases despite many people having multimorbidity. Multimorbidity is associated with lower quality of life, higher healthcare costs, poorer care coordination, and care less focused on what matters to the person. Existing care was perceived as repetitive because multiple appointments and practitioners covered overlapping processes.
