The umbrella review will synthesize review-level evidence on HAPI risk factors in adult hospitalized patients. The protocol uses a multiphase, multicentre, sequential research design with five phases. The E-Delphi phase will develop consensus on predictors for the PIAHA tool using at least 75% agreement. The qualitative phase will identify context-specific and culturally relevant HAPI risk factors through semi-structured expert interviews. Each study phase functions as a complete study and feeds into the next phase.
