Codesigning preventative strategies with community organisations, delivered by trusted community members and centred on peer bonds and enjoyment, is supported by the evidence base. An evaluation of the digital NHS Health Check pilot found that some users regarded the signposting process as too impersonal. Uptake of the NHS Health Check is lowest in the communities with the greatest cardiovascular disease burden, including socioeconomically deprived neighbourhoods and minoritised ethnic groups. Follow-up support after the NHS Health Check appears under-used, with a pharmacy-based study finding low uptake of recommended follow-up. The NHSHC programme targets adults aged 40–74 who have not been diagnosed with CVD, aiming to assess and manage their cardiovascular risk. To increase reach, NHS Health Check delivery has been extended into community venues such as pharmacies, mosques, and pop-up clinics, using social capital to attract higher-risk individuals.
