The intervention bundle is expected to produce a larger effect than prior notification-only interventions because PCPs are not required to act unilaterally and nurse navigators actively support patient completion of evaluation. After the clinical pathologist delivers a diagnostic summary, the primary care provider assumes full responsibility for all subsequent diagnostic decisions. Nurse navigators make up to three phone contact attempts and up to three secure EHR portal message attempts to reach each patient. Nurse navigators receive daily EHR query reports, verify provider participation, notify providers of eligible patients via EHR message, and obtain verbal informed consent from patients by phone. Nurse navigators educate patients on their results, explain next steps, accommodate laboratory location preferences, and facilitate access to diagnostic services.
