PEEP is set at 4–6 cmH₂O for all patients except those with COPD. During one-lung ventilation, tidal volume is 4–6 mL/kg with a respiratory rate of 10–16 breaths/min. End-tidal CO₂ is maintained at 35–45 mmHg throughout ventilation. During two-lung ventilation, tidal volume is 6–8 mL/kg based on ideal body weight. If significant air leak occurs with applied PEEP, the level is titrated downward to maintain SpO₂ ≥94%.
