The intervention exposed operational gaps that were not visible in the quantitative analysis. Many staff did not know how to arrange transport or interpretation support when patients requested it. Staff without rebooking access had to pass rescheduling requests to another team, adding delay and duplication. Reminder calls revealed that existing cancellation channels were often ineffective. Some staff appeared to skip patients whose names did not appear obviously British.
