Total knee arthroplasty was the most common arthroplasty procedure in the event group. The arthroplasty rate of 6% in this BAP cohort appears higher than the 3.2% prevalence in the general Swedish population, though figures are not age-matched. The study measured severe osteoarthritis through joint replacement surgery due to osteoarthritis. Osteoarthritis was the study outcome and was defined by ICD-10 codes M15 to M19 in primary or specialist care. Only one total hip arthroplasty due to osteoarthritis was identified, which was unexpectedly low given that hip OA is a known complication in transfemoral amputees. Undetected hip OA is likely in this cohort because the study did not perform systematic hip symptom screening or radiographs. Contralateral knee OA was the most common first arthroplasty event, occurring in 4 of 5 patients whose first event was arthroplasty. The article describes joint deterioration as a feedback loop involving poor lubrication, inflammation, enzymatic degradation, and impaired rebuilding. Joint symptoms after age 55 are framed as the visible result of long-running biological changes rather than ordinary aging alone. Knee osteoarthritis appears more strongl…
