Prior research by the study group found that PENG block with liposomal bupivacaine provided analgesia lasting up to 60 hours and sometimes beyond 72 hours. Single-injection PENG block with liposomal bupivacaine is selected specifically because it offers prolonged analgesia while minimizing motor impairment. Successful PENG block placement is identified by an anechoic area between the iliopectineal eminence and anterior inferior iliac spine, with upward displacement of the iliacus muscle tendon. All PENG blocks in the trial are ultrasound-guided using the SONIMAGE HS1 Ultrasound System from Konica Minolta and performed by anesthesiologists with more than 10 years of experience. Local anesthetic systemic toxicity is identified as the main foreseeable risk of the PENG block procedure. Potential PENG block side effects include nerve damage, functional impairment, vascular injury, hematoma, injection-site pain, and quadriceps weakness.
