Participants are instructed to use PLD for at least 60 minutes daily while resting supine. The PLD device allows separate pressure settings for foot, lower leg, thigh, and trunk within a 20-60 mm Hg range. Clinical efficacy of PLD after LVA had not previously been tested in a randomized controlled trial. Earlier exploratory lymphoscintigraphy studies suggested PLD could facilitate tracer transport under low-pressure conditions without serious device-related adverse events in mild lower extremity lymphoedema. The investigational PLD system is designed to provide anatomically directed mechanical stimulation aligned with physiological lymphatic pathways. Participants assigned to PLD use the device from the day of surgery until Day 183 in addition to standard postoperative care.
