Revisional surgery has historically been used for inadequate post-bariatric weight outcomes but has important limitations. BARI-STEP defined poor weight loss as less than 20% weight loss at least one year after primary gastric bypass or sleeve gastrectomy. The trial included both suboptimal initial weight loss and weight regain after initial loss. Poor initial weight loss or later regain is clinically important because postsurgical benefits depend strongly on weight loss.
