POTS is a severe form of postural hypotension strongly associated with very low vitamin B1 levels, and high-dose B1 supplementation can greatly improve it. Postural hypotension occurs when the autonomic nervous system fails to adapt to standing, causing the brain to temporarily lose adequate oxygen supply and resulting in dizziness or fainting. Recent intense exercise, dehydration, poor sleep, and alcohol consumption are common factors that combine to trigger a postural hypotension episode. A simple adrenal self-test measures blood pressure sitting then immediately after standing; a systolic drop of 6–10 or more points suggests adrenal gland weakness.
