PP4M Intervention

The intervention included a computerised multimorbidity template and individualised patient preparation materials. Patient preparation materials were tailored to individuals, explained recent test results, and prompted patients to consider what they wanted to discuss. The template included prompts about what matters most to the patient, quality of life, pain, function, and mental health. The PP4M intervention was designed to support a comprehensive annual review that considers all of a patient's long-term conditions together. The intended PP4M model used two extended consultations instead of multiple shorter disease-specific appointments.
