In practice, preoperative fitness is often estimated through unstructured clinical interviews, an approach that neither accurately measures fitness nor predicts complications. The true incremental prognostic value of the 6MWT beyond routine clinical factors remains uncertain. Preoperative cardiopulmonary fitness is a well-established determinant of postoperative risk. Research priority-setting initiatives identify improved preoperative risk stratification as a leading unmet need.
