Sustainability varied across practices, with some normalising the process, some planning modifications, and some achieving limited change. Administrative and IT fit varied by practice, including booking longer appointments, coordinating reviews, and adapting recall systems. The study took place in general practices in Bristol, Southampton, and Staffordshire. All practices used EMIS electronic patient records, had at least 5000 registered patients, and had access to Ardens templates. Practices in more deprived areas were prioritised because multimorbidity is more prevalent there.
