Using an 80-90% decline from peak PCT values can reduce antibiotic treatment duration by a median of 2 days and may provide mortality benefit. The approximately twofold PCT increase approach has not previously been tested in a randomized controlled trial. Observational studies suggest that about a twofold PCT increase predicts infection with a mean area under the curve of 0.85. The MOSES study showed prognostic value for PCT kinetics. An early rise in PCT over a defined period may indicate a new ICU infection. KIPRUN applies procalcitonin kinetics to antibiotic initiation rather than antibiotic discontinuation.
