The uniform-dose arm delivers 27 Gy to the whole prostate or clinical target volume in two fractions. The de-escalated arm gives a lower whole-gland dose plus focal tumor boost. Radiotherapy for localized prostate cancer has shifted toward hypofractionated and ultra-hypofractionated SBRT schedules supported by level 1 evidence. Two-fraction schedules are supported by HDR brachytherapy data and the low alpha/beta ratio of prostate cancer.
