An R/I ratio of at least 0.5 indicates recruitable alveoli and triggers a higher PEEP strategy; below 0.5 indicates non-recruitable lungs and lower PEEP. The recruitment-to-inflation ratio is calculated from a bedside one-breath decremental PEEP manoeuvre that takes less than 5 minutes. Airway closure is assessed using a low-flow inflation airway pressure-time curve before measuring recruitability. In the experimental group, if alveoli are recruitable, PEEP is set at 15 cm H2O to maximise recruitment; if non-recruitable, PEEP is set between 8 and 12 cm H2O.
